I would consult John's pages on the Z80 board and Memory board.

The serial version of the images use the dual S100 computers board at 19200 baud rate.

Results in the banked CPM starting successfully; I've verified this sequence is reliable on my hardware and verified that without presetting the lower 16K page to 0x01 it hangs during boot.
