To answer your second question, No, no modifications to the monitor or boot rom is necessary, As you pointed out in a previous post, once the first 12 sectors are loaded up and CPMLDR.COM starts to execute all CHS to LBA requests are done within CPMLDR.COM and CPM3.SYS.
