I had the same problem months ago,
Never did solve the issue.  It's been on hold.

Select drive- ok
Select format - ok.  Write protect led illuminates.
Step in - ok
Read a track or sector - system halts with halt led illuminated.

These symptoms occur with both a 5.25 and a 3.5" drive using different cables and power source.
